Reporting to the Vice President, Quality and Patient Safety, the System Director, Care Management is responsible for management and leadership of Care Management and Social Work Services for UHSH, Chenango Memorial and Delaware Valley Hospitals. With a focus on care coordination, length of stay reduction, case management, and throughput. The System Director oversees the related processes to ensure the effectiveness and appropriateness of services.
The System Director, Care Management is also responsible for providing appropriate training and education to increase knowledge of care management concepts and trends to streamline activities and improve patient outcomes, as well as keep current with Federal and State regulatory issues/requirements as they pertain to care management.
Position requires a bachelor's degree in human services, nursing, or related field along with valid NYS registered license if applicable, or eligible as well as five years of relevant clinical experience. Working knowledge of patient registration, billing, and health insurance practices also required. Master's degree and certification in case management (CCM) are preferred.
