Medical Services Deputy Project Director

Function Of Work:
The MFP Deputy Project Manager will report to the Project Director. The Deputy Project Director will assist the Project Director in the, coordination, and operation of the Rhode to Home MFP Demonstration Project. The Deputy Project Director will assist in ensuring that all MFP participants are able to live safely and securely in the community while guiding the project staff to meet all five Operational Protocol benchmarks. The Deputy Project Director will supervise Care Coordination services such as Transition Assessment, Housing, Transition Coordinators and Medical/Therapy consultants. The Deputy Project Manager will step in for and serve as the project director when the project Director is not available or able to perform his/her duties.

APPLICATION INSTRUCTIONS

The MFP Deputy Project Manager will report to the Project Director. The Deputy Project Director will assist the Project Director in the, coordination, and operation of the Rhode to Home MFP Demonstration Project. The Deputy Project Director will supervise Care Coordination services such as Transition Assessment, Housing, Transition Coordinators and Medical/Therapy consultants. The Deputy Project Manager will step in for and serve as the project director when the project Director is not available or able to perform his/her duties.

Assist in developing and implementing strategies for community outreach and education; identification of target population; comprehensive assessment and care plan development; facilitation
of housing and community based long term care services; oversight and monitoring; implement metrics to evaluate effectiveness and costs of care.
Assist in monitoring project performance, expenditures, and meeting re-balancing LTC goals,
Assist in work with stakeholders throughout the demonstration,
Assist in the design of strategies and programs to re-balance the long term care delivery and financing systems compatible with Medicaid, and;
Assist in preparing MFP and other LTC reports for CMS, the State or other interested parties.

Qualifications:

Required (minimum)
Ø At a minimum, the Deputy Project Director will have five to seven years’ experience with the Medicaid program and in the LTC industry working to rebalance the LTC delivery and financing system.

Ø The Deputy Project Director will have strong commitment to the project goal of enabling persons who would otherwise reside in an institutional setting to successfully live in a community setting.

Ø The ideal candidate will have progressive experience with LTC systems designed to serve individuals with complex medical, behavioral health and co-morbid conditions; and have expertise and experience conducting health care analytic tasks.

Ø The Deputy Project Director will have a Master Degree in health care or a degree related to this demonstration.

Ø Hiring of the Deputy Project Director will be done in consultation with the Director of Office of Institutional/Community Services and Supports (OICSSS) and/or his/her designee.
All other duties as assigned.
